OD – RIGHT EYE    ALWAYS ON THE TOP  (oculus dextra)

OS – LEFT EYE   ALWAYS ON THE BOTTOM (oculus sinistra)

CF – count fingers, estimate from how many feeet

eg : CF @ 2 ft – counting fingers from 2 feet, but not from farther

HM– HAND MOTION, has to be able to tell direction : horizontal or vertical

LP – LIGHT PERCEPTION ONLY, NOTE IF WITH PROJECTION TO UP/R/L/DOWN    OR NOT

NLP – NO LIGHT PERCEPTION

Have patient stand at line, cover left eye first, let patient read with right eye first. Always mark right eye’s acuity first, then left eye. Mark the smallest line patient still can read. If patient misses more than 1 character on a line, mark line above as visual acuity.
